Description:

Identity documents (ID Cards) are issued to Namibian citizens or permanent residence permit holders who are 16 years or older. The ID Card serves as a legal form of identity when dealing with public and private institutions.

You can apply for ID Cards at any of the Ministry of Home Affairs and Immigration (MHAI) offices.

Requirements:

Application:

• Application Form (Pink Form)
• Original/full Birth Certficate
• Original Citizenship Certificate (Only People born outside Namibia)
• Marriage Certificate (Married Women only)
• Original Permanent Resident Permit (Permanent Residents only)
• Old Namibian ID (Only applicants in possession of an old ID)
• Fingerprints (taken in office)
• ID Photo (taken in office)

Alterations:

• Application for Alteration / Duplicate Form (White form)
• Original or copy of ID
• Documents to act as proof of requested change [Original (not handed in) and copy]

Change of Data:

• Data update Form (Blue form)
• Original or copy of SWA ID
• Original (not handed in) and copy of Birth Certificate
• Fingerprints (taken in office)
• ID Photo (taken in office)

Correction:

• Application for Alteration / Duplicate Form (White form)
• Original or copy of ID
• Documents to act as proof of requested change [Original (not handed in) and copy]

Duplicate:

• Application for Alteration / Duplicate Form (White form)
• Original (not handed in) and copy of Birth Certificate or Citizenship Certificate or PRP
• Fingerprints (taken in office)
• ID Photo (taken in office)

Enquiries:

• Receipt of Application
• Proof of Identity
• Referral Letter (If applicable)

Relevant good quality certified photocopies of the above documents should be submitted with your application.

Procedure:

You can apply for your National ID Card at any office of the Ministry of Home Affairs and Immigration (MHAI). All applications are sent to the MHAI head office in Windhoek for further processing. Your fingerprints will be matched with those already on record or entered into the National Population Registration System. Your application will then be processed and once issued, your ID card will be forwarded to the office where you applied for you to collect.

Correcting errors on your ID Card:

If the ID card issued to you has errors pertaining to your personal particulars, the Ministry of Home Affairs and Immigration will replace the ID Card at no cost. Application should be submitted at any of the Ministries offices to make the necessary corrections. As part of the application, proof of the error should be provided, showing the correct information.

Re-issuing / Replacement of your ID Card:

ID Card can be re-issued in the following cases:

• If you have married and want to assume the surname of your spouse. A certified copy of the marriage certificate should be submitted and the required fee should be paid.
• If you are a women and want to apply for a new ID Card in the name of any previous surname. Documentary proof should be provided showing you are entitled to use that surname. The required fee for issuance should be paid.
• If you're ID card was stolen, damaged or lost. Additional documentation should be provided and the required fee will need to be paid for the duplicate card.

• Application (first time issuing): Free
• Correction: Free
• Change of Data: Free
• Duplication issuing: N$50.00
• Alteration issuing: N$50.00
